Program Information

Camp Dates: June 7 - 11, 2023

Application Process: The following steps must be completed before your child can officially be accepted as a participant for the Aspen Summer Adventure

If you have any technical issues please contact Rosemary White at Rosemary@ShiningStarsFoundation.org or 970-726-8009. For any health-related questions, please contact our Medical Director, Dr. Larry McCleary, at elm481@gmail.com or 720-840-6528.

  1. Click here to complete and submit our health and social history form as soon as possible. If you already filled this application out, you can skip this step and fill out a short medical update form: Click here.

  2. Click here to complete a waiver and medical consent form for your child as soon as possible

  3. Email the following to Dr. Larry at Elm481@gmail.com

    • Copy of your child’s insurance card (Front and Back)

    • Photo of your child

  4. If your child is on chemotherapy, please send a recent CBC and ANC 4- 5 days prior to attending camp. Email this information to Dr. Larry McCleary at Elm481@gmail.com
