2025 Aspen Summer Adventure
Application & Program Information

Program Dates: June 4 - 8, 2025

Application Process: The following steps must be completed before your child can officially be accepted as a participant for our Winter Games Program:

1. Click here to complete and submit our Aspen Summer Adventure Health & History Form.

2. Click here to complete a waiver and medical consent form.

3. Click here to view the Aspen Summer Adventure Guidelines form. Please read and complete the entire form with your child. Both the parent/guardian AND child must sign this form together.

4. Click here to upload a copy of your insurance card (front and back). If you are not able to upload the insurance card, please email it to Office@ShiningStarsFoundation.org

5. Click here to upload a recent photo of your child. If you are not able to upload the insurance card, please email it to Office@ShiningStarsFoundation.org
