2025 Aspen Winter Games
Application & Program Information

Program Dates: March 21 - 28, 2025

Application Process: The following steps must be completed before your child can officially be accepted as a participant for our Winter Games Program:

  1. Click here to complete and submit our Winter Games Health & History Form.

  2. Click here to complete a waiver and medical consent form.

  3. Click here to view the Aspen Winter Games Guidelines form. Please read and complete the entire form with your child. Both the parent/guardian AND child must sign this form together.

  4. Click here to upload a copy of your insurance card (front and back). If you are not able to upload the insurance card, please email it to WinterGames@ShiningStarsFoundation.org

  5. Click here to upload a recent photo of your child. If you are not able to upload the photo, please email it to WinterGames@ShiningStarsFoundation.org

  6. If your child is on chemotherapy, please send a recent CBC and ANC 4- 5 days prior to attending camp. Email this information to Dr. Larry McCleary at Elm481@gmail.com
